Self-referral to Physiotherapy
From Monday 3rd February 2014, the self-referral phone line and drop in clinics for this service at Stobhill Hospital and Springburn Health Centre are being removed.
In its place there are "Patient Completed Self Referral Forms" which you complete yourself and either hand in or post to your local physiotherapy department.
Please collect a copy of the Self Referral form from reception at the surgery.
